In accordance with federal law, the County Board is responsible for preparing Form 2441 to be sent to the Secretary. The County and each municipal corporation will use the County Form 2441, as well as any related information provided by the County, to prepare and send Form 2421 electronically to the Secretary. All information submitted on any Form 2441 or Form 2421 are confidential. Payments related to certain taxes made by certain counties and municipal corporations must be included on Form 2421.
